Before your first visit: We prefer to have your insurance information (if applicable) before your first appointment so we may verify your benefits and move forward without unexpected hurdles. We also have the option for you to pick up or print your new patient packet to save time at your initial visit. We are available to help navigate this process and assist with these documents. We are here for you!
Your first visit: Initial visits will be scheduled for 1 hour. At this first visit, we ask that you arrive 15-20 minutes early to fill out a new patient packet, if you have not already. If we do not already have it, we ask that you bring an ID, your referral (script from your physician) and your insurance card, if applicable. During this visit, we gather your medical history, perform an evaluation and may walk you through selected performance measures. We take time to hear from you what's really important and your main goals. By the end of the first visit, we work with you to put together a plan for how we can best guide you to reach your goals. During the first visit, the priority is getting a good baseline and understanding of where we are and where we need to go for your needs and goals.
Follow-up visits: After your initial visit, follow-up visits are scheduled for about 45 to 60 minutes. We prefer you wear closed-toed shoe wear to your appointments for safety, as well as clothing that does not restrict your movement.
Please do not enter the facility if:
• You have been diagnosed with active COVID-19
• You have been instructed to actively isolate/ quarantine by a health care provider
• You have symptoms of illness that are not normal to you, COVID-19 or otherwise
Face coverings over one’s mouth and nose are recommended upon entry and throughout your time in our office.
You may also be asked to wash your hands or use hand sanitizer upon entry. We have telehealth visits available if you are at high risk for complications due to COVID-19 and would like to further minimize risk. Please ask us for more information and scheduling.
At Connect Movement Center LLC, the provider will be wearing a face covering and sanitizes/washes hands between clients. Equipment used is cleaned in between clients and we utilize air filters. Our providers and staff have been fully vaccinated against COVID-19. Please feel free to ask for more details on how we are working to keep everyone safe.
